Auto-debit required. All memberships have a 12-month minimum.
Memberships are non-transferable. Unused benefits do not roll over month-to-month.
Unlimited services are subject to medical discretion and reasonable scheduling.
Discounts apply to regular service prices only and cannot be combined with other promotions.
Cancellation requires written notice 30 days prior to next billing. Early cancellation: $50 fee and forfeiture of remaining month’s benefits.
At the time of enrollment, you are expected to pay for the first and last month of membership.
